New Online Application Instructions:

The family must reside within the Skyridge boundaries, or approval through the Out of Boundary Request must be received.

The application must be filled out by the custodial parent or legal guardian. Custody paperwork with judge’s signature or Alpine District Power of Attorney must be provided if applicable.

Student must be withdrawn from previous school before starting classes at Skyridge.

ALL supporting documents must be uploaded BEFORE the application can be processed.
The Following Documents are REQUIRED:
We recommend scanning all documents before starting the online application. Documents need to be saved as PDF files.

Original birth certificate. Small wallet size certificate cannot be accepted.

Proof of Residency (Only a LEHI CITY UTILITY BILL, LEASE, or PURCHASE AGREEMENT is accepted). Residence with a separate family requires an additional notarized form.

Copy of immunization verification from Dr. Office.

(If Applicable) Divorce Decree establishing custody with judges signature or Alpine District Power of Attorney.
