New Online Application Instructions:

The family must reside within the Skyridge boundaries, or approval through the Out of Boundary Request must be received.

The application must be filled out by the custodial parent or legal guardian.

Student must be withdrawn from previous school before starting classes at Skyridge.

The application WILL NOT BE PROCESSED until ALL supporting documents are uploaded.
The Following Documents are REQUIRED:

Original birth certificate.

Proof of Residency (LEHI CITY UTILITY BILL, LEASE or PURCHASE AGREEMENT accepted). If residing with another family, an additional notarized form is required.

Copy of immunizations.

(If Applicable) official court custody documents or Alpine District Power of Attorney must be provided.
